PLAYER AGREEMENT REGISTRATION FORM - IF REGISTERING WITHOUT PAYMENT OR MAILING CHECK
Please read and complete this information to register a player for whom you are mailing a check to RIHA for Player Payment.
TERMS & CONDITIONS: This is an agreement between the registered Player and the Player's Parents or Guardians (hereinafter referred to as "Parent") and the Regional Ice Hockey Association (hereinafter referred to as RIHA). This agreement constitutes a binding Agreement between the Registered Player, Parent and RIHA. The Player and/or Parent agree to pay RIHA the discounted Personal Player fee of $1,800.00 in full prior to Sept. 15, 2024. A Payment Plan consisting of 3 payments $600 each on July 1st, Aug 1st, and Sept 1st, 2024 or Payment in Full prior to Sept 15, 2024 will be considered compliant. All payments not paid in full by Sept 15, 2024 will forfeit the discount of $300.00 and will pay $2,100 in full prior to any participation or tryout. In the event the Player participates in tryouts for his/her respective High School and does not make the team, the Player Fee will be refunded in full minus a $100 tryout fee. Note there is a 10% discount of the total fee for a Parent's second Player (use Coupon Code 2NDPLAYER when paying online) and an additional 5% discount for each subsequent Player (use Coupon Code SIBLING#3). Player fee may be reduced by the discounts if applicable when paying by check. Please note that once a player has made a team and the team plays a game, there will be NO REFUNDS, partial or otherwise. It is important to understand that RIHA is only a fundraising organization and playing time is a coaching decision. In addition, Parent understands that one parent will be required to work the gate at least one time at a Home Game during the season. Fundraising is mandatory for RIHA as a 501c3 Non-Profit organization and while Parent participation in all fundraisers is not mandatory it is strongly encouraged to help lower the Player Fees.
PAYMENT PLAN: The Payment Plan consists of three (3) monthly payments of $600 each prior to the 15th of the month beginning on July 1, 2024, Aug 1, 2024 and Sept 1, 2024.
PAYMENT OPTIONS: RIHA accepts cash, checks payable to RIHA and credit card payments through our website RIHANJ.org. Please do not send cash in the mail. If paying by check please give your check to a RIHA Representative or mail to above address with this form AND complete Player Agreement online.
I have read these terms and conditions and agree to them entirely and understand that my Player cannot participate or tryout until full payment of the Player Fee is received by RIHA.
